➢ Our goal is to provide high-quality treatment plans, including clear information so that
you are able to make an informed choice about your treatment.
➢ We encourage you to discuss your treatment plan with us, and you have the right to
withdraw from this service at any stage.

➢ You are absolutely welcome to bring a support person with you, especially if they will
be closely involved in helping you work towards your health goals.
➢ In accordance with the Privacy Act all information will be kept confidential and
accessed only by staff from The Healthstyle Doctor in accordance with our scope of
service. 
➢ It is understood that you give us permission to write back to your referring General
Practitioner or specialist, and share your information with your normal GP if you have self
referred, unless specified by you otherwise.
If you do not wish to have information shared with your GP or specialist, we respect this,
and will keep all information confidential unless this compromises your safety or
wellbeing.

Information for your lifestyle consultation

Please let us know if you:
➢ Are pregnant, trying to get pregnant or become pregnant during treatment
➢ Have any allergies
➢ Have any serious medical conditions or physical or mental health conditions
that may affect our assessment and treatment of you.
➢ Have any changes in your medical condition during the time of your treatment.
➢ Have any concerns about your proposed treatment plan or problems resulting from
your treatment plan.
➢ Have any particular cultural needs or requirements that we should be aware of.
Lifestyle consultation fees
➢ Payment is required BEFORE your initial appointment, either by credit card (via online
booking system), or internet banking. Follow-up appointments will be invoiced and must be paid within 2 weeks.
➢ Any appointment cancellations or reschedules must be made at least 48h in advance, any cancellations /reschedules made within 48h will incur a fee of 50% of the appointment cost
➢If you do not attend an appointment a 50% ‘did not attend’ fee will be charged.
➢Appointment costs:
★ First appointment $359 (50 mins)

★ Follow-up appointment $245 (40 mins)  
★ Follow-up appointment $149 (20 mins)

CONSENT FOR TREATMENT

I consent to treatment and understand that a full verbal explanation of my treatment plan will be given at the time of consultation, and a summary will be emailed to me within 72 hours of my appointment.
I understand I have the right to seek a second opinion at any time. I agree to keep The Healthstyle Doctor informed of any changes in my medical condition or circumstances that could affect my care.

PRIVACY CONSENT
In accordance with the Privacy Act, all information recorded in your health records will be kept strictly confidential. All personnel in this practice are required by law to maintain strict patient confidentiality. We can at times use a medical dictation service called Heidi, this does not include any identifying features. Records are stored in a secure patient management system on our computer, or in hard copy. Under the Privacy Act you have the right of access to, and correction of, your personal information held by this practice. No information will be given to a third party without your consent unless required

by law.
I understand that if this data is collected for audit purposes, no information will be collected that would allow me to be personally identified.

RIGHT TO SUPPORT AND ADVOCACY CONSENT
I understand that I am entitled to have a support person(s) with me when undergoing assessment or treatment, except where safety may be compromised or other patients' rights may be unreasonably infringed on. I also understand I am entitled to advocacy services and can access these through
Health and Disability Service (Ph 0800 11 22 33)

CONSENT FOR PAYMENT
I understand that: If I fail to attend my appointment or cancel without 24 hours' notice, I will be charged 50% of the appointment fee.
If debt recovery service is required to recover my debt, I will be held liable for any debt recovery fees,
PLUS an administration fee of $50
